IMMUNIZATIONS
6th grade-
REQUIREMENTS-
INCOMING 6TH GRADE STUDENTS MUST HAVE THE MENINGITIS (MCV4)AND TETANUS BOOSTER (TDAP).
BOTH ARE GIVEN AT AGE 11-12.
**IMMUNIZATIONS ARE DUE BEFORE THE FIRST DAY OF SCHOOL.

SCREENINGS
The Indiana State Department of Health Mandates health screenings at the following grade levels:
VISION: Grades 1, 3, 5, and 8
(1st Grade students have MCT screening done by volunteer Optometrists)
HEARING: Grades 1, 4, 7, 10
Screenings are also done if the parent and/or teacher request it. School nurses perform the screenings. The screening is not a diagnostic tool; it simply indicates that further medical evaluation is needed. If you receive a screening referral for your child, follow up with his/her physician. Please be sure to notify the nurse of the results so we are aware of any changes in their health status. Parents or legal guardians who have a religious objection to these screenings should contact the school nurse.
MEDICATION
All medications given at school must have a consent form signed by a parent/guardian. Parents must provide the medication in the original bottle, no baggies or unmarked containers will be accepted. Please access the Medication Consent Forms by clicking on the Forms menu.
Students who carry inhalers or epipens need to have an Asthma or Alllergy Plan completed and signed by both parent/guardian and health care provider. Please access these form by clicking on the Forms menu.
Please remember to pick up any unused medication from the school at the end of the school year. Medication cannot carry over to the next school year.

EXPERIENCE
I have been a registered nurse for 25 years. I am a Purdue graduate. I started my career in the Intensive Care and Open Heart units at Porter Memorial Hospital. I transitioned to Emergency Medicine and became an E.M.T. to further my education. I had a number of certifications to care for adult and pediatric trauma patients including Advanced Cardiac Life Support. I also worked for 8 years as a certified Oncology/Chemotherapy nurse. When I did some substitute school nursing, I discovered I really enjoyed the environment and especially the students.
